Now
Where am I in my life right now?
What is this?
Four boxes about your life today. Fill in what fits. Write or draw. There's no wrong answer.
Bring this back next session. We'll use it in your PATH plan later in the program.
My Team
Who's in my corner?
What is this?
The people and places around you. Write names in each box. Draw if you prefer. There's no wrong answer.
Circle the 3 people you would call first if you needed help.
Halfway check-in
4 weeks done. 5 weeks to go. How's it going for you?
We're halfway through the PATH Leadership Group program. We want to know what's working for you - and what's not.
This is not a test. There are no wrong answers.
Take your time. Write or draw. Ask for help if you want it.
You keep this in your folder. Next week we'll talk about what the whole group says - but never your own answers, unless you say it's OK.
What's the most important thing you've learned about leadership in the first half of this program?
Who has been most helpful to you in this program so far? What did they do?
What's harder than you expected?
It's OK if nothing has been hard. It's OK if a lot has been hard. Just write what's true for you.
Looking at the topics ahead - is there something you'd like more time on?
One thing about your PATH plan that's becoming clearer.
What happens next?
This sheet is yours. Keep it in your folder. You can come back to it at home any time.
You can leave any question blank. You can change your answer later. There are no wrong answers.
The whole group's answers - no names, no specifics - help DAS run the second half of the program better. If lots of people ask for more time on something, we make space for it.
Want to talk about anything you wrote? Your facilitator is here for you. So is your co-facilitator.

Ask for an interpreter.
Many of these services can get you an Aboriginal language interpreter through the NT Aboriginal Interpreter Service (NTAIS). Call 1800 334 944. Just ask.
Bring a support person.
A support worker, family member, friend, or DAS advocate can come with you. You don't have to do this alone.
You don't have to build your team alone.
DAS, NPY Women's Council Tjungu, Congress Disability Services, Tangentyere, Carers NT and CatholicCare NT are all funded to help you find and stay connected with the right services. Call any of them.
