One small step you've taken since we started.
Since our first session - what is one small step you have taken?
Big or small. In the group or at home. If it's true, it counts.
Examples (not the right answers):
- I asked a question in front of the group.
- I sat next to someone new.
- I said no when I needed to.
- I came every week.

What is a PATH Plan?
A PATH Plan is a map for your future.
- A 2 to 3 hour day with you, your team, and DAS facilitators
- We make a big poster together - words and pictures about your future
- You decide who comes. You decide what is on the map
- You keep the map
- PATH stands for Planning Alternative Tomorrows with Hope
This is the day you have been working towards - a celebration as well as a planning day. The map shape is on the next slide, and the COLOUR CODED PATH print is in your folder.

- Write your big goal at the top of the ladder.
- Step 3 is the closest step. Step 1 is the first thing you do.
- Make your steps small. Smaller than you think.
- You can do this with your support worker, family, or Mandy.
- Bring this ladder to your PATH Plan day.

Getting ready for your PATH Plan.
When?
A morning, an afternoon, a whole day. School holidays? Pension week? Weekend?
Who do you want to invite?
Look back at your My Team page. Family, friends, support workers, NDIS provider, neighbours.
What colours?
Your favourites. Footy team colours. The colour you painted your room.
What do you want to bring?
A photo. A piece of music. Something that grounds you. Bush plum, footy scarf, your dog.
Even rough is fine - we can fix details over the next two weeks. We need WHEN before you leave today. Mandy can scribe.

Looking after yourself for your PATH Plan day.
Before the day
Good sleep. Eat breakfast. Know the time and place. Get there early. We help you sign the consent and photo form.
During the day
It's your map. Take your time. It's okay to pause, have a drink, or ask us to slow down.
After the day
Rest. Look at your map. Tell someone about it. Pick your first small step.
Pick one thing in each row to actually do. Circle it on your sheet. Mandy can scribe.
