What is a PATH plan?
A PATH plan is a way of working out:
- Where you want to go in your life.
- What is going on for you right now.
- Who can help you get there.
- What small steps you can take.
Who does the plan?
You do — with the people who back you up.
Family. Friends. Support workers. People you work with. Anyone you choose. DAS facilitators help. We ask the questions and write down what you say. The plan is yours.
What does it look like? When?
What it is
- A big colourful map on paper.
- With words and pictures.
- You take it home to keep.
When you do it
- After our 8 sessions together.
- We book a day for you and your team.
- Just one day. One plan. Yours.
